What does integrated care look like in practice?<\/p>\n\n\n\n

Sometimes it starts with a free orthopedic examination of a child.<\/strong><\/p>\n\n\n\n

In the Fakulteta neighborhood every week dozens of children get access to free orthopedic examinations and ultrasound of the hip joints. For many families, this is an opportunity for medical care that they would not otherwise have reached.<\/p>\n\n\n\n

But this is only part of the change.<\/strong><\/p>\n\n\n\n

With the SOLACE-CEE<\/strong> project we are creating an integrated care model that puts the person in the center. Along with health services, children and families living in insecure homes gain access to a sensory room, social and psychological support, and a safe space where they can find help, understanding and perspective.<\/p>\n\n\n\n

For people living in extremely precarious housing conditions, the challenges are rarely just housing. They relate to health, child development, education, mental well-being and social inclusion.<\/p>\n\n\n\n

That is why integrated care is important \u2013 because it does not look at problems individually, but builds a system of support around the person.<\/strong> We believe that sustainable change starts with affordable services, partnerships across sectors and care that reaches people where they are.<\/p>\n","protected":false},"excerpt":{"rendered":"
